Providing safe, personalized dental care starts with understanding your overall health. Certain medical conditions, medications, vitamins, and supplements can affect the way we plan your treatment, so we’ll ask you to provide a complete and accurate health history before your first visit.
Please complete the form below before your scheduled appointment and include a current list of medications and dosages. You can email us the form as an attachment or bring a completed copy with you on the day of your appointment.
Thorough Care, With Technology to Match
Your first comprehensive exam gives Dr. Do a detailed look at more than just your teeth. We use digital X-rays and a 3D CBCT scan to evaluate your teeth, gums, jaw, bone, and surrounding structures, along with a non-invasive oral cancer screening.
This technology helps us catch things that may not be visible during a standard exam and gives Dr. Do the information he needs to make thoughtful recommendations based on the full picture.
Once everything is complete, we’ll review the findings with you and answer any questions before discussing next steps.

Once your exam is complete, Dr. Do will sit down with you to review what we found and talk through your options. If treatment is recommended, we can prioritize what needs attention first and plan the rest around your needs, comfort, schedule, and budget.
We also want you to feel comfortable throughout your visit. If you’re nervous, have had a difficult dental experience in the past, or simply need us to slow things down, let us know.
